Accidents usually have more than one cause...but I think the takeaway is the self-driving is not safe yet partially because we other HUMAN drivers are not perfect. In fact many said the self-driving cars would perform better than imperfect humans, and clearly , with one death with so few of these cars on the road....this is not true.
