The thing is, once AI replaces surgeons (likely within the next century), a scenario where the book's events play out could theoretically happen. AI would be so advanced that it could understand how human life works on a subnuclear level and alter humans in a way to revive dead ones and make living ones immortal. They could then torture or enslave these humans to do what it pleases with them provided they have access to everything else on earth, which in all honesty, if they are doing surgery on humans, its likely they control nearly everything, since medicine is one of the last fields predicted to be replaced by AI.
