In a situation where heavy load falls of from truck then it is assuming it is a highway. (because stuff dosn't fall of slow vehicles.) Then the self driving cars are required to maintain a safe distance which they do even now. Hence they can stop and instruct cars behind for the same with communicating with them. Also different lanes have different speeds so if you have to change lanes in a situation explained as above, you go to the faster lane instructing other cars behind to adjust and make way for this car.
