I'm taking a course right now that has heavy emphasis on treatment access for individuals living in these sorts of conditions.
In addition to the distance barrier, when they arrive to a treatment facility there is no guarantee that they will be seen upon arrival. Long lines, lack of supplies etc means that someone may make the journey to receive treatment and have it be all for not. This means that some people won't even bothering trying to get treatment because it can be an entire day or two of them not working which can be critical to feed their family.
It's a very sad situation. ):
